HOW DOES A STUDENT ENROLL? | | |
| 1. | Contact us for a tour of the school and ask for enrollment papers or download these from our website. | | 2. | Complete the "Application for Admission" sheets and "Parent Code of Conduct" form (click here for printable copies) and return them to the school along with the following:
- $30 non-refundable testing/evaluation fee for each new student
- A copy of the student's birth certificate
- A copy of the student's most recent report card
- A current "Commonwealth of Kentucky Immunization Certificate"
- A copy of the most recent school physical exam
- A copy of a current eye exam for students entering Kindergarten

Old Union's School Board and administrators will set up a meeting with the child and parents where the student and family will be interviewed and the student will be tested/evaluated. After this meeting, the family will be notified of the student's enrollment status at Old Union School. A $30 enrollment fee is due only upon a student's acceptance to OUS. |
